Scooter accidents are part of daily life for doctors in Phuket, and many of the riders I treat were not wearing a helmet or protective clothing. Even small falls can cause deep grazes that need careful cleaning, and I always check for injuries that are not obvious at first.
We treat scooter injuries in Phuket all year round, and the pattern is familiar. Steep hills between the beaches, sand and gravel on bends, sudden downpours that turn the tarmac slick, and first-time riders in flip-flops and swimwear all play a part. Most falls happen at low speed and cause grazes, cuts, burns and sprains that heal well with proper care. A smaller number cause head, spine or internal injuries that need a hospital straight away, so knowing which is which matters from the first minute.
Right after the fall: hospital or clinic?
First make the scene safe: switch off the engine, move out of traffic if you can walk, and warn oncoming riders. If someone has neck or back pain, numbness, or was knocked out, do not move them unless they are in danger, and leave the helmet on unless it is stopping them breathing. What we check when you come in after a crash
Adrenaline hides pain, so the doctor examines more than the obvious graze. We check your head, neck, chest, abdomen and each limb, test movement, strength and feeling, and look for tenderness over bones. For a head knock, we assess memory, balance, pupils and coordination. Wounds are cleaned under local anaesthetic where needed, and your tetanus vaccination status is checked. Pain relief is usually paracetamol, with an anti-inflammatory such as ibuprofen if it is safe for you. A sprain is supported with a bandage or splint, and if a fracture is possible, we refer you to a hospital for an X-ray rather than guess. Road rash, burns and cuts: why cleaning matters
Road rash is a friction injury that grinds sand, tarmac and grit into the skin. If the grit stays, it can leave permanent dark marks and raise the risk of infection in Phuket’s heat, so we clean it thoroughly, often under local anaesthetic, and use non-stick dressings that keep the wound slightly moist. Exhaust burns on the inner calf need cooling and a burn dressing. Deeper cuts may need stitches, glue or strips. Symptoms that can appear hours later
Some injuries announce themselves the next morning. After a head knock, a worsening headache, drowsiness, repeated vomiting, confusion or unusual behaviour in the first 48 hours needs urgent review. Pain in the upper left belly or the tip of the left shoulder can signal a bruised or torn spleen. A stiff, sore neck after being thrown forward is common whiplash, but numbness or weakness is not. Ribs that hurt more with each breath may be cracked. Have someone stay with you for the first 24 hours after any head injury, avoid alcohol and sleeping tablets, and do not ride.
Insurance, licences and helmets
Before relying on your travel insurance, read the policy wording. Many policies exclude motorbike accidents if the rider did not hold a valid motorcycle licence, meaning one that covers motorcycles in your home country, usually with an International Driving Permit that also includes them, or a Thai motorcycle licence. Many also exclude accidents where the rider or passenger was not wearing a helmet, where alcohol was involved, or where the engine was above a certain size. Helmets are compulsory for riders and passengers under Thai law. Check your own policy, because the cost of hospital care after a serious crash can be very high without cover.
Paperwork for insurers and the police
Insurers usually want a medical report describing the injuries and treatment, itemised invoices, photos of the injuries and the scene, and your rental agreement. If another vehicle or person was involved, report the accident to the police; the Tourist Police can be reached on 1155. When to see a doctor
See a doctor on the same day for any graze larger than your palm, any cut that gapes, a burn with blisters, a joint that is swelling or cannot bear weight properly, or any bump to the head. Return quickly if a wound becomes red, hot and more painful after day two, or if new symptoms appear. Skip the clinic and call 1669, or go directly to a hospital emergency department, for loss of consciousness even for a moment, confusion, repeated vomiting, a seizure, clear fluid from the nose or ears, a suspected fracture, neck or back pain with tingling or weakness, chest or abdominal pain, breathlessness, heavy bleeding, or feeling faint, cold and clammy.
Prevention and early detection
Wear a properly fastened full-face or open-face helmet on every ride, however short, and make sure your passenger does too. Long sleeves, long trousers and closed shoes turn many injuries into bruises instead of road rash. Ride slowly on hills, bends and wet roads, watch for sand near beaches, and never ride after drinking. If you have not ridden a motorbike before, Phuket’s traffic is not the place to learn. For early detection after any fall, check yourself again the next morning: new pain, swelling, headache or drowsiness deserves a doctor’s look.

I feel fine after the fall. Do I still need a check-up?
If you hit your head, fell at speed or have more than a small graze, yes. Adrenaline can mask pain for hours, and some injuries, such as concussion or a bruised spleen, show themselves later. A check within the first day also creates a medical record from the time of the accident, which insurers value.
Will my travel insurance cover a scooter accident in Thailand?
Only if your policy includes it and you met its conditions. Many policies exclude riders without a valid motorcycle licence, riders or passengers without a helmet, and accidents involving alcohol, and some limit engine size. Read your policy wording.
