Minor Surgery in Phuket: Small Procedures Under Local Anaesthetic

Minor surgery in Phuket, such as draining an abscess, treating an ingrown toenail, removing a cyst or taking out sea urchin spines, is done under local anaesthetic at Takecare Clinic, 24/7 in Bangtao, Kata and Patong. You stay awake, most procedures are quick, and you go back to your hotel the same day. Anything needing a general anaesthetic is referred to hospital specialists.

I carry out small procedures such as draining abscesses, removing ingrown toenails and closing clean cuts, which often saves people a hospital visit. Before anything, I explain what will happen, how long it takes and how to care for the area afterwards.

The minor procedures we do most often in Phuket are shaped by the island itself. Heat and sweat encourage boils and abscesses, days in flip-flops and wet sand aggravate ingrown toenails, and rocky beaches leave people with sea urchin spines, splinters and the occasional fish hook from a fishing trip. Alongside these, residents and long-stay visitors ask us to remove cysts, skin tags and small fatty lumps. This page explains what a clinic procedure involves, how to time it around swimming and flights, and when a hospital surgeon is the better choice.

What minor surgery at a clinic covers

Minor surgery here means short procedures on the skin and the tissue just beneath it, done with local anaesthetic so that only the small area being treated is numb. You stay awake, can usually drive or ride a taxi home afterwards, and need no fasting. It does not include cosmetic operations, anything needing sedation or a general anaesthetic, large or deep lumps, or work close to important nerves, tendons or the eye. For those, and for anything the doctor is not certain can be done safely in a clinic, we refer you to a hospital specialist rather than stretching what a treatment room can offer.

Common procedures and what recovery looks like

ProcedureWhy it is doneUsual recovery
Abscess incision and drainageA painful pocket of pus that antibiotics alone rarely clearPain eases quickly; dressing checks for several days
Partial toenail removalAn ingrown edge causing repeated pain or infectionAbout 2-4 weeks to heal fully
Sea urchin spine or splinter removalFragments causing pain, swelling or infectionUsually a few days
Fish hook removalA barbed hook embedded in skinSame day, plus a tetanus check
Cyst or small lipoma excisionA lump that is troublesome or repeatedly inflamedStitches out after 7-14 days
Skin tag removalTags that catch on clothing, straps or jewelleryHeals within one to two weeks

What the procedure is like, from consent to dressing

Before

The doctor examines the problem and asks about allergies, especially to local anaesthetics, antiseptics or plasters, and about blood-thinning medicines such as aspirin, clopidogrel, warfarin or apixaban, which can increase bleeding. Tell us if you are pregnant, have diabetes or have had problems with healing or raised scars. We explain what will be done, the likely result, the small risks of bleeding, infection and scarring, and the alternatives, including doing nothing. You sign a consent form only once your questions are answered, and we confirm the price on WhatsApp or at the desk before starting.

During

After cleaning the skin, the doctor injects a local anaesthetic, usually lidocaine, through a very fine needle. It stings for a few seconds, then the area goes numb within minutes and stays numb for an hour or two. From then on you should feel pressure or pulling but not sharp pain; say so at once if you do, and more anaesthetic can be given. An abscess is opened and the pus washed out, sometimes with a soft gauze packing left in to let it drain. A cyst is removed whole, with its lining, to lower the chance of it returning, and the skin is closed with stitches.

After

You leave with a dressing, written aftercare instructions and a plan for follow-up. Paracetamol usually controls the discomfort once the numbness wears off, and raising a treated foot helps with throbbing. Antibiotics are not routine after drainage and are used mainly when infection is spreading into the surrounding skin or you have diabetes or a weakened immune system. For wound checks and when stitches come out, see our page on stitches removal.

Sea urchin spines and other island foreign bodies

Sea urchins are common on rocky shores and reef flats around Phuket. Their spines are brittle, break off in the skin and often leave a dark purple mark, which is dye rather than a retained spine. Soaking the area in water as hot as you can comfortably tolerate helps the pain. Many small, shallow fragments work their way out or are absorbed over days to weeks, but spines that are deep, near a joint, causing persistent pain or showing infection should be removed. A spine that has entered a finger or toe joint may need imaging and a hospital specialist. Grit left in road rash after a fall is another foreign body worth removing early, which our page on motorbike accident care covers.

Timing a planned procedure around your holiday

Urgent problems such as an abscess, an infected toenail or an embedded hook should be treated when they happen. Planned procedures, such as removing a cyst or skin tag, need more thought. Most leave a wound that must stay out of the sea and pools for about one to two weeks, and partial toenail removal takes longer. Stitches need removing at the right time, and flying a day or two after a procedure is usually fine, but a wound on the leg or foot swells on long flights. The best time is often early in a long stay, or at home. An inflamed cyst is usually drained first and removed only once the inflammation has settled, several weeks later.

When to see a doctor

See a doctor the same day for a boil or lump that is rapidly getting larger, hotter and more painful, redness spreading around a toenail, a spine or splinter you cannot remove, or any procedure site that becomes more painful after the second day. A fever, red streaks tracking up the limb, or pus from a stitched wound suggest infection; our page on skin infection treatment explains the next steps. Call 1669 or go to a hospital emergency department for bleeding that does not stop after 15 minutes of firm pressure, a high fever with confusion or a racing heart, or swelling and pain around the anus or groin with fever, which can signal a deeper abscess.

See a doctor if: a lump is painful and hot, a toenail is weeping or swollen, a spine is lodged near a joint, you have diabetes and any wound on your foot, or a procedure site is getting worse instead of better. Call 1669 for uncontrolled bleeding or high fever with confusion.

Prevention and early detection

Cut toenails straight across rather than rounding the corners, and dry your feet well after swimming. Wear reef shoes on rocky beaches and when wading over coral or rocks, which protects against urchins and cuts alike. Shower after sweaty days and avoid squeezing boils, which can push infection deeper. For early detection, note any lump that grows, changes colour, bleeds or becomes painful, and any mole that changes shape or colour. Those need a proper assessment, and a changing mole should be checked by a doctor rather than simply removed as a skin tag would be.

Prevention point: treat a red, sore toenail corner early with warm soaks, clean dry feet and open footwear. An ingrown nail caught in the first few days can often settle without any procedure at all.

Arranging a procedure with Takecare Clinic

We are open around the clock at the Bangtao (Cherngtalay) clinic, the Kata-Karon clinic and the Patong clinic. For urgent problems, see a walk-in doctor in Phuket at any hour; for planned procedures, send us a photo on WhatsApp or call +66 81 718 9080 to arrange a suitable time. Procedures themselves need the lighting and sterile set-up of a treatment room, but assessments and follow-up dressing checks can be done during a doctor hotel visit in Phuket. Most travel insurers accept direct billing from us for covered treatment, and afterwards you receive an itemised invoice and a short report of the procedure.

Summary

Minor surgery in Phuket covers short procedures under local anaesthetic: draining abscesses, treating ingrown toenails, removing sea urchin spines, fish hooks, cysts and skin tags. You stay awake, go home the same day and follow a simple aftercare plan. Urgent problems should be treated promptly, while planned procedures are best timed so the wound can heal before you swim or fly. The practical takeaway: treat hot, painful lumps and sore nail corners early, and keep planned removals for a time when you can stay out of the water.

“Plan your swimming around the procedure. Most small wounds need to stay out of the sea and pools for several days to heal cleanly.”

Frequently asked questions

Will I be awake, and will it hurt?

Yes, you stay awake. The local anaesthetic stings for a few seconds as it goes in, and after that most people feel only pressure or pulling. If anything feels sharp, tell the doctor and more anaesthetic can be given. Once it wears off, the area is usually sore rather than painful, and paracetamol is enough for most people.

How soon can I swim after a minor procedure?

Usually not until the wound has sealed, which is about one to two weeks for a stitched excision or a drained abscess and often longer after toenail surgery. Seawater and pools can carry bacteria into an open wound. The doctor gives you a specific timeline based on the procedure and the site.

Can I walk in, or does minor surgery need booking?

Urgent procedures, such as draining an abscess or removing a spine or fish hook, can usually be done when you walk in, at any hour. Planned procedures, such as cyst removal, are easier to schedule, so message us first and we can confirm the price and a convenient time.

Can the doctor do the procedure in my hotel room?

Assessment, yes: a doctor can visit you anywhere in Phuket, 24/7, look at the problem and advise. The procedure itself is safer in our treatment room, with proper lighting, sterile instruments and space to work. Follow-up dressing changes and wound checks can often be done back at your hotel.

Will travel insurance pay for minor surgery?

Urgent treatment for a new problem, such as an abscess, an infected toenail or an embedded spine, is often covered. Planned removal of a long-standing cyst, lipoma or skin tag is usually treated as elective and not covered. Check your policy wording. We offer direct billing with most travel insurers and confirm the price on WhatsApp beforehand.

Will a removed lump be checked under a microscope?

If there is any doubt about what a lump is, it should be, and the doctor discusses this with you before the procedure. For anything suspicious, such as a changing mole, we often advise removal at home or by a hospital dermatologist, so that you receive the result and any further treatment without the complication of having left Thailand.
