I write medical certificates for travellers who fell ill and missed a flight, a tour or work, and for insurance claims after an illness abroad. A certificate is only useful if it is accurate, so I document what I actually find during the examination and when the illness started.

What a doctor can put on your certificate
A medical certificate is a signed statement from a licensed doctor describing an examination on a specific date. Ours normally includes your name and passport number, the date and time you were seen, the relevant findings, the period during which the doctor advises rest or restricted activity, and the doctor’s signature, medical licence number and the clinic stamp. The diagnosis appears only if you agree. Many employers are content with wording such as unfit for work due to a medical condition, which keeps your health details private.
What we will not write
We do not backdate. If you were ill before coming in, the certificate can record when you report symptoms started, clearly marked as your own account. We also cannot certify fitness for an activity we have not assessed, sign a form without seeing you, or promise that a particular employer, school, insurer, embassy or government office will accept the document.
Matching the certificate to its purpose

Driving licences, visas and work permits in Thailand
Thai offices that handle driving licences, some visa categories and work permits commonly ask for a medical certificate from a licensed doctor, and many only accept one issued a short time before the application. Some forms ask the doctor to confirm that you do not have particular conditions, or to record specific tests. Requirements change and can differ between offices, so confirm the current version with the office, your employer or your visa agent first. We complete every form truthfully from our findings, and the decision to accept it rests with the office.
Too unwell to travel: certificates for insurance claims
When illness disrupts a holiday, the certificate often decides whether a claim succeeds. Insurers typically want the date you were examined, what was wrong, and the specific dates you could not fly, dive or join a booked excursion. When to see a doctor
If you want a sick note because of an illness, some symptoms need assessment today: a fever lasting beyond two days or arriving with a rash, vomiting that stops you keeping fluids down, passing very little urine, severe abdominal pain, or a headache unlike your usual ones. In Phuket, fever after mosquito bites always deserves a doctor’s review, because dengue is common on the island. Skip the clinic and call 1669, Thailand’s emergency medical service, or go straight to a hospital emergency department, for chest pain, difficulty breathing, fainting, sudden weakness of the face or limbs, slurred speech, a seizure or heavy bleeding.
Prevention and early detection
The simplest way to avoid certificate problems is to plan for them. Read your insurance policy and your employer’s sick-leave rules before you fly, so you know whether they need a note from the first day of illness. Carry your passport or a photo of it, because the certificate has to match your identity document. For a licence or permit, collect the official form first and book the medical once you know how recent it must be. A yearly fitness examination can also pick up problems early, such as blood pressure that has crept up unnoticed.

Frequently asked questions
Can the certificate cover days before I saw the doctor?
Not as a doctor’s finding, because a certificate cannot be backdated. We can note, clearly labelled as your own account, when your symptoms began and what happened before the visit, alongside what the doctor found on the day. Some insurers and employers accept that, but being seen on the first day of illness is always stronger.
How quickly will I have the certificate in my hand?
For a sick note or an insurance certificate linked to a current illness, usually before you leave, and most walk-in visits take under 30 minutes. Fitness forms that need blood tests take longer, although common tests run in our on-site laboratory with same-day results. If a form asks for a test we do not perform, we tell you at the start.
Is an appointment necessary, or can I just turn up?
You can walk in at any of our three clinics at any hour, and most people are seen within minutes. For official forms, a WhatsApp message first is worthwhile, because we can check the form, tell you what to bring and confirm the price before your visit. For a simple sick note, walking in is perfectly fine.
I am too ill to leave my room. Can a doctor write the certificate at my hotel?
Yes. Our doctors visit hotels, villas and condos anywhere in Phuket, 24/7. The doctor examines you, treats you if needed and issues the certificate during the same visit, so the document reflects your condition at the time. It also gives your insurer a clear record from the day you were unwell.
Will my travel insurance cover the visit and the certificate?
It depends on your policy. When the visit is for an illness or injury your insurance covers, the consultation is often claimable, and we offer direct billing with most travel insurers. A certificate needed purely for a licence or work permit is usually a personal cost. We confirm the price on WhatsApp before your visit and itemise every receipt.
Will my employer, school or a Thai government office accept this certificate?
Our certificates are signed by a licensed doctor, dated, stamped and carry the doctor’s licence number, which is what most organisations look for. Every employer, school, insurer and office sets its own rules, though, and we cannot guarantee acceptance. Check whether they need a particular form, language or validity period, and bring that form with you.
